Should I include a platform at the top?

Always, when the stairs meet a door or pathway. A platform (landing) of at least 36 inches deep at exterior doors meets most building codes and provides space for opening the door safely. For interior stairs, the landing is the floor of the destination room.

Rebar in concrete stairs?

Recommended for exterior stairs subject to freeze-thaw cycles. Typical: #4 bars in each step, 12 inches on center each direction. Helps prevent corner spalling. Interior stairs in stable soil can use fiber reinforcement instead.

Slope the platform for drainage?

Yes, for exterior stairs. Slope the top platform 1/4 inch per foot away from any building entrance. Same applies to step treads if water might pool. Otherwise water collects and freeze-thaw damages the surface.
